Save your spot today to ensure the perfect class for you! Send this form with your non-refundable registration fee of $15 per family to: Tarilyn’s Dance Studio 14238 Pierce Plaza Omaha, NE 68144. E-mail confirmations will be sent out August 5th. You’ll be able to pick up your confirmation packet at one of our Open Houses the end of August.
Classes fill up quickly and we can not hold any places without a registration fee.

Please enclose your non-refundable $15 registration fee (per family) and return to the office NO LATER than August 1st. No spaces will be held without a registration fee. There is a $25 fee for returned checks. (Make copies for additional students).
